Informed Consent for Self-Care and Skill-Building Group

The self care and skill building trauma group is an educational and supportive psychotherapy group for people who have survived stress and trauma. This group will be educational, exploring different aspects of self care as it relates to enhancing your sense of balance, stabilization, and safety. The focus and purpose of this group is to help you learn self care and stress management techniques that will set the stage for trauma recovery work, in individual as well as group therapy.

As a participant in this group I agree to focus on my self care and what supports or presents as obstacles to following through with the personal self-care plan.   I understand that this is not a trauma debriefing group or a process group. I agree to keep my sharing focused on myself and to refrain from giving feeback to other people.  I understand the leaders of the group will provide support to maintain appropriate boundaries. 

As a member I agree to engage in the techniques and practices taught in the group, as a way to help me stay present in the group, and practice what I am learning.  I will make an effort to follow through with my self-care plan, so as to be able to contribute to the group session the following week.

I agree to attend all sessions, and if there is an emergency or some unavoidable reason that I can not attend the group, I acknowledge that I am still responsible for payment even if I do not attend.

I agree that group membership and all discussions are strictly  confidential. I agree not to share any information regarding anything that happens in the group outside of the group,  except in therapy as it relates to my process and progress.
